## About the Role

Our client, a home healthcare agency conveniently located near Richmond Avenue and Wilcrest Drive in Houston, is seeking a compassionate and organized Part-Time Bilingual Case Manager to support its Home and Community-based Services (HCS) and Child-Specific programs.

Location: Houston, TX (Richmond & Wilcrest area)

Schedule: Part-time, at least 3 days per week (Monday-Friday), * Bilingual (required)

 * Experience in case management, home health, HCS, IDD, or community-based healthcare preferred.
 * Strong communication, organizational, and interpersonal skills.
 * Proficient with Microsoft Office and electronic documentation systems.
 * Professional, dependable, detail-oriented, and committed to quality client care.

## Description

* Coordinate services for individuals enrolled in the HCS and Child-Specific programs.
 * Conduct client assessments and assist with the development and monitoring of individualized service plans.
 * Serve as a liaison between clients, families, providers, and community resources.
 * Maintain accurate documentation and case records.
 * Ensure compliance with program requirements, state regulations, and agency policies.
 * Advocate for clients while providing exceptional customer service and support.
 * Manage office operations and coordinate staff to ensure efficient daily business functions.
