## About the Role

* Proven hospital experience with exposure to Level II or Level I Trauma Centers; ICU or PICU experience is highly desirable.
 * Strong background in acute care settings such as emergency medicine, inpatient units, or outpatient clinics.
 * Experience working with children, individuals with disabilities or substance use disorders; familiarity with mental health counseling is a plus.
 * Knowledge of medical terminology, physiology, diagnostic evaluation techniques, and clinical case management principles.
 * Proficiency in EMR/EHR systems and electronic health records management; understanding of HIPAA compliance is essential.
 * Excellent client communication skills with the ability to work effectively with diverse cultural backgrounds and language needs; bilingual fluency is required.
 * Ability to prioritize tasks efficiently in a fast-paced environment while maintaining high-quality patient service and care standards.
 * Relevant certifications such as Basic Life Support (BLS), CPR, and addiction counseling credentials are preferred.

## Description

We are seeking a dynamic and compassionate Bi-lingual Case Manager to join our dedicated healthcare team. In this vital role, you will serve as a bridge between patients, families, and healthcare providers, ensuring seamless coordination of care across diverse settings. Your bilingual skills will enable you to effectively communicate with individuals from various cultural backgrounds, fostering trust and understanding. This position offers an exciting opportunity to make a meaningful impact on patient outcomes by providing personalized case management within a fast-paced, patient-centered environment. You will be instrumental in guiding patients through complex healthcare journeys, from intake through discharge planning and community resource coordination., * Conduct comprehensive patient assessments, including medical history, social needs, and behavioral health considerations, utilizing your physiology knowledge and clinical expertise.

 * Develop individualized care plans that incorporate hospital experience, acute care, ICU, emergency medicine, and post-acute care insights to optimize patient recovery and well-being.
 * Coordinate inpatient and outpatient services, ensuring smooth transitions of care across settings such as Level I Trauma Centers, PICUs (Pediatric Intensive Care Units), and community clinics.
 * Collaborate with multidisciplinary teams to facilitate diagnostic evaluations, behavioral health interventions, addiction counseling, and mental health support tailored to each patient's unique needs.
 * Manage electronic health records (EHR) efficiently while maintaining strict adherence to HIPAA regulations to protect patient confidentiality.
 * Engage with patients' families and caregivers to provide education, emotional support, and discharge planning that aligns with their cultural backgrounds and individual preferences.
 * Connect patients with community resources related to social work, managed care, disability services, and behavioral health programs to promote holistic recovery.
