## Description

Case Manager (FT) (Bi-lingual Spanish)

The Opportunity

The Case Manager will deliver youth-focused gun violence prevention and community violence intervention programming for FCA's Empowerment Through Wellness to Prevent Gun Violence Initiative.

Rate: $27.47/ An Hour

Responsibilities

 + Work as an onsite case manager and member of the team to support youth charged with gun-related offenses.

 + Provide individualized case management for all participants.

 + Make appropriate cross-system referrals i

 + Life Skills: Harm reduction, parenting, healthy habits, financial literacy, and planning.

 + Manage, build and create relationships with local agencies and service providers.

 + Assist youth in navigating systems including providing court advocacy and transportation to medical appointments.

 + Work with the court staff/resource coordinators, mentors and therapeutic counselor and assist them in providing case management services to their high-risk program participants.

 + Coordinate case planning/ service delivery with assigned court staff, including Resource Coordinators, and make appropriate cross-system referrals.

 + Engage with other community-based service providers as appropriate to ensure a wraparound approach.

 + Be available to support the team in emergencies regarding incidents with staff or participants.

 + Adhere to all documentation and database requirements.
