## About the Role

Do you have experience in Youth services?, Do you have a Master's degree in social work?, Bachelors Degree in Social Work required, Master's Degree in Social Work highly preferred, with licensure in the State of Minnesota required.
Previous Experience:

 + Experience in mental health highly preferred
 + Experience using electronic health records preferred
 + Proficient in Microsoft Office Products including Word and Outlook
 + Proficient in Microsoft Office Products including Word, Outlook, and Teams.
 + Able to effectively and efficiently communicate in written and verbal form.
 + Ability to smoothly and rapidly shift between dissimilar tasks.
 + Ability to respond appropriately in times of mental health crisis.

## Description

We are looking for a Social Worker to join our team within our youth Partial Hospitalization Program. As a Social Worker, you will collaborate closely with psychiatric medical providers and therapists on the patient's primary treatment team to ensure a cohesive and effective plan of care., * Assessment and Planning: Conduct effective assessments and suicide risk evaluations, develop treatment and discharge plans, and lead educational or therapeutic groups.

 * Patient and Family Communication: Maintain open lines of communication with patients and their families to ensure they are actively involved in treatment decisions.
 * Care Coordination: Coordinate care with outpatient providers during patient admissions and discharges to ensure continuity of care.
 * Team Facilitation: Facilitate weekly treatment team meetings, creating and updating master treatment plans based on team consensus.
 * Clinical Contribution: Bring a unique clinical perspective to the interdisciplinary team, drawing on training in family systems and social work theory
