meet regularly with assigned participants to monitor, evaluate, and document progress in accordance with program requirements. + Conduct initial intake meetings and assess participant needs to inform service planning and resource development. + Facilitate timely referrals to appropriate levels of care or external providers in collaboration with the clinical team. + Ensure prompt connection to emergency services or crisis intervention resources when required. + Collaborate with the Post Response Supervisor and designated clinical staff to interpret assessment results, reassess service needs, and identify appropriate community-based supports aligned with participant goals. + Work with supervisory and program staff to evaluate program effectiveness, identify areas for improvement, and adjust workflows and caseloads as needed. + Coordinate communication with partner agencies and multidisciplinary providers, including clinical staff, probation officers, and other stakeholders. + Establish and maintain professional, collaborative relationships with internal team members and external partners, including Co-Response and law enforcement agencies when appropriate. + Accurately document all participant interactions, services, and progress toward treatment goals in accordance with program standards. + Maintain up-to-date records, including data, notes, and required documentation, using electronic health record systems and other tools;
