A clinical background is a plus, and experience in marketing, networking, and relationship building is strongly preferred., * Minimum of one year of experience working with seniors or within the senior care industry. * Knowledge of Medicaid Waiver programs and VA Aid & Attendance benefits. * Familiarity with senior living communities and adult foster care homes within the Western/ Mid-Michigan territory. * Experience in marketing, networking, business development, or relationship management. * Clinical background preferred but not required. Skills & Attributes * Compassionate, client-centered approach. * Excellent interpersonal and relationship-building abilities. * Ability to guide families through complex and emotional decisions with professionalism and empathy. * Strong organizational and time-management skills. * Exceptional follow-up and communication skills. * Proficiency with Google Workspace (Gmail, Calendar, Drive, Docs, and Sheets). * Ability to work independently while contributing positively to a collaborative team environment. ## Description As a Care Transition Manager, you will build and maintain referral partnerships, guide families through senior care placement decisions, and provide hands-on support throughout the transition process. This role combines community outreach, client advocacy, and ongoing case management., * Identify and cultivate referral partnerships with hospitals, rehabilitation facilities, physicians' offices, senior communities, home health agencies, hospice organizations, and other related professionals. * Meet regularly with social workers, discharge planners, and healthcare professionals who assist seniors transitioning from home or healthcare settings. * Attend networking events and community functions to build reciprocal referral relationships and identify valuable resources for clients and families. * Represent CarePatrol professionally within the community while promoting our mission and services. Client Support & Advising * Conduct in-depth care discovery meetings with seniors and their families. * Assess client needs and provide guidance regarding appropriate care options. * Schedule and conduct tours of senior living communities and care homes. * Support families throughout the placement process, helping minimize stress and confusion. * Maintain consistent follow-up communication with families and referral partners to ensure a smooth transition and positive outcomes.
