High School diploma or equivalent. 2. One (1) year of related administrative experience., 1. Occasionally lift and/or move up to 25 lbs. 2. Ability to adapt to changing priorities. ## Description *This role focuses on performing activities associated with evaluating, developing, recommending, implementing, and carrying out the policies and procedures related to the delivery of high quality patient care. In addition, this role focuses on performing the following Care Navigation related duties include: Focuses on the patient's needs and helps guide the patient through the healthcare system and works to overcome obstacles that are in the way of the patient receiving the care and treatment they require. An organizational related support or service (administrative or clerical) role or a role that focuses on support of daily business activities (e.g., technical, clinical, non-clinical) operating in a "hands on" environment. The majority of time is spent in the delivery of support services or activities, typically under supervision. An experienced level role that requires basic knowledge of job procedures and tools obtained through work experience and may require vocational or technical education. Works under moderate supervision, problems are typically of a routine nature, but may at times require interpretation or deviation from standard procedures, and communicates information that requires some explanation or interpretation. Job Overview This position ensures continuous, efficient and smooth operations performing administrative and clerical support for assigned care managers. Handles matters of a routine nature to assist patients in their transition of care., **Duties and Responsibilities:** The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned. 1. Performs general office functions for assigned department including, but not limited to typing, filing, email/mail correspondence, purchase/order supplies and equipment. 2. Uses and maintains office equipment such as copy machines, fax machines, etc. 3. Maintains confidentiality in all office matters. 4. Answers phones in a professional manner as required for screening, referring, follow-up calls, taking messages and managing voice mail. 5. Greets visitors in a friendly, professional manner and direct to appropriate person/place. 6. Provides departmental administrative and clinical support and participates in special projects as needed. 7. Assists with phone triage for Continuity of Care. 8. Establishes relationships and communicates with designated contacts for patients transitioning to an alternate level of care. 9. Prints daily census reports, serves as liaison with skilled nursing facilities, visiting nurse agencies, infusion companies, hospices, etc. 10. Faxes necessary paperwork; assists COC staff with copying and faxing of medical records. 11. Rounds on inpatient units to deliver notifications required by CMS to patients. 12. Assists COC staff with post-acute care planning as needed such as calling referral sources, family members, transportation, etc. 13. Enters documentation into EMR including but not limited to insurance authorizations, denials, or other important UR communications. 14. Documents in the EMR, including flagging of critical information (ex. TB rule-out), as requested. 15. Serves as primary facilitator of insurance appeal process. 16.
