Proficiency in Microsoft Word and Excel preferred. ## Description The Transition Coordinator (TC), reporting to the Epilepsy Programs Manager (EPM), plays a crucial role in facilitating the smooth transition of patients from pediatric to adult healthcare settings. This position involves monitoring, tracking and reporting on the transition process, addressing barriers, and maintaining accurate documentation in the Electronic Health Record (EHR). The TC serves as an advocate for transitioning patients within multiple healthcare systems and community resources, ensuring their unique needs are met.
