Bilingual KNOWLEDGE, SKILLS AND ABILITIES: Ability to establish and maintain effective working relationships with families, students, school staff and community agencies. Ability to conduct effective home visits. Must possess strong computer skills including the ability to understand, develop and utilize early warning systems. Strong problem-solving skills. Ability to motivate students. Ability to maintain basic files, records, databases and spreadsheets. Strong oral and written communication skills. ## Description Responsible for resolving problems that interfere with student progress and achievement including excessive absenteeism, lack of health/personal care, homelessness, and limited parent engagement., * Develop and utilize early warning systems to identify students who are most at risk due to absenteeism, lack of health/personal care, homelessness, and poor parent involvement. * Collaborate with school social workers, guidance counselors, teachers and school leaders to develop systemic strategies to identify and support students who are frequently absent. * Partner with Social Worker to provide access to: + Health Services: vision, dental, hearing, medical and mental health services. + Personal Services: community referral for clothing, food, housing and medical assistance. + Parent Involvement: parent participation in school-based conferences, Title I plan and parental support in completing and complying with school policy and procedures. + Resources in the community and school. * Develop relationships with parents/guardians, community organizations, and area agencies. * Conduct home visits. * Provides mentoring, on-going emotional support and personal contact with at-risk students. * Develop and implement individual intervention strategies to increase student attendance including developing the social and emotional skills of students. * Work with school staff to develop and promote models for school connectedness. * Work with school and community stakeholder to plan transitions (Starting school, 5th grade to middle school, 8th to high school) and monitor student transition to help them successfully adapt to the rigor of high school. * Track the progress of individual students. * Communicate regularly with parents and teachers of students identified as being at risk. * Provide training to teachers on support measures that work with identified youth and families. * Develop partnerships with community organizations to in developing and implementing enrichment, mentoring and tutoring programs. * Conduct and analyze on-going formative and summative evaluation data of program effectiveness. * Provide and/or submit reports as scheduled and requested. * Maintain effective and efficient recordkeeping procedures * Maintain records of parent and student contact PHYSICAL REQUIREMENTS: Light Work: Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently as needed to move objects. TERMS OF EMPLOYMENT: Salary and benefits shall be paid consistent with the District's approved compensation plan. Length of the work year and hours of employment shall be those established by the District. EVALUATION: Performance of this job will be evaluated in accordance with provisions of the Board's policy on evaluation of personnel.
