The Application Analyst I - Epic Healthy Planet/Compass Rose role supports patient coordination and population health initiatives, enabling care teams to proactively manage patient journeys, improve outcomes, and drive operational excellence through advanced Epic applications and workflow optimization., * Design, build, and maintain Epic Healthy Planet/Compass Rose components supporting patient coordination workflows

 * Configure care coordination tools such as registries, care gaps, outreach workflows, and patient tracking

 * Support ServiceNow ticket queues, troubleshooting and resolving end-user issues efficiently

 * Analyze complex population health and care coordination needs, translating them into Epic solutions

 * Identify system gaps in patient coordination processes and recommend scalable enhancements

 * Collaborate with clinical, operational, and IT stakeholders to optimize care management workflows

 * Contribute to integration efforts involving data exchange and interoperability within Epic modules

 * Participate in technical discussions to improve system design and patient engagement strategies

 * Document workflows, configurations, and system updates to support ongoing optimization and compliance
