In this role, you'll coordinate comprehensive care for patients with complex needs, ensuring they have access to the services and support necessary to improve health outcomes and quality of life.  * CHW (Community Health Worker) training/certification preferred
 * Strong understanding of healthcare systems and community resources
 * Excellent communication, interpersonal, and organizational skills
 * Ability to work independently while collaborating effectively with a team
 * Comfortable using electronic health records and care management software
 * Willingness and ability to travel within Marin County

## Description

* Patient Assessment: Conduct thorough evaluations of patients' health, social, and care needs
 * Care Coordination: Facilitate seamless communication between healthcare providers, patients, and families
 * Care Planning: Develop and implement individualized, person-centered care plans
 * Resource Management: Connect patients with community-based services and support programs
 * Monitoring & Follow-Up: Track patient progress, adjust care plans as needed, and conduct follow-up visits or calls
 * Education & Support: Educate patients and families about conditions, treatment plans, and resources
 * Data Management: Maintain accurate documentation in compliance with regulatory standards
 * Collaboration: Work closely with healthcare teams to ensure coordinated and effective care
