Ability to clear a Department of Human Services background screening. ## Description Mobile Case Manager, Mobile Case Manager: Meet youth where they are - beyond the doors of the Rochester Youth Shelter. 180 Degrees believes support shouldn't stop at the shelter door. As a Mobile Case Manager (MCM), you're the bridge between shelter and community, the consistent connection that helps youth stay stable and supported long after they've exited our doors. You're out in the community, in the spaces where youth naturally gather, making sure no one falls through the cracks during a critical transition. This role sits at the heart of 180 Degrees' continuity-of-care model. You provide prevention-focused, community-based support and structured aftercare that keeps youth connected to stability as they move from shelter back into daily life. You collaborate closely with shelter staff and fellow Mobile Case Managers across the organization, ensuring every youth experiences a consistent, unified journey no matter where they are or what program they're in. You'll operate with independence in the field but you're part of the Shelter team. This role is grounded in strong supervision, teamwork, and constant connection back to shelter operations. What you'll actually do: Serve as a consistent point of connection between shelter operations and the community Provide prevention-focused, community-based case management to youth exiting shelter Maintain structured aftercare services that ensure continuity of care during a critical transition Build consistent outreach presence in spaces where youth naturally gather Collaborate closely with shelter staff and other Mobile Case Managers to maintain alignment as youth move between programs and communities Work independently in the field while staying grounded in supervision and Shelter teamwork Maintain strong, consistent connection to shelter operations and the broader continuity-of-care model Support planned and unplanned discharges, ensuring appropriate transition planning and continuity of care. Maintain professional boundaries, confidentiality, and ethical practice Keep clear, accurate documentation to support youth planning and progress This role is for you if: You believe support shouldn't end when shelter does. This is a full-time, onsite position (40 hours/week). No direct reports. Travel to community partners and appointments as required. Work Environment: This role moves with the rhythm of a program that never closes. 180 Degrees operates 24/7/365. Crisis doesn't run on a 9-to-5, and neither do we. Case Managers participate in the on-call rotation, and flexibility to work extended hours for events and special projects is part of showing up fully for this work.
