Preferred Qualifications + Epic Healthy Planet certification and recent implementation experience. + Experience with: + Care Everywhere + Reporting Workbench + Radar Dashboards + SlicerDicer + Cogito + Knowledge of HEDIS, CMS quality measures, and risk-adjustment programs. + Experience in large healthcare systems or integrated delivery networks., Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors. ## Description We are seeking an experienced Epic Healthy Planet Analyst to support a growing pipeline of population health initiatives and strategic healthcare transformation projects. This role will be responsible for the design, build, optimization, implementation, and support of Epic Healthy Planet solutions that improve patient outcomes, care gap management, quality performance, and population health programs across the organization. The ideal candidate will partner with clinical, operational, and executive stakeholders to develop population health workflows, analytics, registries, and care management solutions that support value-based care objectives and organizational goals.
