The schedule requires working from 10:00 a.m. to 6:30 p.m. Pacific Time, regardless of the state of residence, and includes rotating weekend shifts. This role focuses on assessing post-hospital needs, coordinating referrals and services, managing documentation in electronic health records, and communicating clearly with patients, families, providers, and payers. The position requires strong organization, attention to detail, and the ability to work efficiently in a fast-paced, fully remote healthcare environment.
