## Description

Carlty Gardens, professionally owned and managed by Ohana Ventures, is a premier senior living community that is seeking an experienced and energetic full time Behavioral Support Manager to join our Leadership team. The Behavioral Support Manager is responsible for evaluation, development, implementation, oversight, and continuous monitoring of individualized behavioral supports, coordination of mental health services, and compliance with all applicable OARs and contract requirements.

Behavioral Support Manager responsibilities:

 * Completion of person-centered evaluations
 * Development of and implementation of Individualized Behavioral training and review of Individual Behavioral Plans
 * Completion of Behavior Plans specific to each Individual
 * Individual-specific coaching and group teaching for direct care staff
 * Monthly review of Individual Behavior Plans.
 * Assist with creating Policies & Procedures relevant to Behavioral Health needs and train for implementation
 * Oversee managed risk agreements
 * Track individual behavior plan progress
