This position is responsible for document imaging, medical record analysis, data integrity, and patient record management while ensuring compliance with healthcare regulations and privacy standards., Document Imaging * Prepare, scan, and quality review medical records for electronic storage. * Upload and index records into document management and electronic medical record (EMR) systems. * Ensure records are accurately processed and available for clinical and coding teams. Health Information Management * Analyze medical records for completeness and assign provider deficiencies as needed. * Support missing documentation requests and provider queries. * Perform chart reconciliation, retrieval, filing, and record maintenance. * Assist with data integrity activities, including chart corrections and duplicate medical record resolution. Vital Statistics & Compliance * Complete birth and death certificate documentation in accordance with state regulations. * Maintain patient confidentiality and uphold all HIPAA and privacy requirements. * Ensure compliance with facility policies and regulatory standards.
