Preferred * Experience supporting interdisciplinary healthcare teams and clinic operations. * Experience with organizational change management, end-user training, healthcare technology implementations, and workflow design. * Experience in PACE, Medicare, Medicaid, managed care, home health, senior care, or other integrated care delivery models. Licenses/Certifications: Healthcare informatics, project management, EHR application, or training certifications (e.g., CAHIMS, CPHIMS, CAPM, PMP, CPTM, or vendor-specific EHR certifications) are strongly preferred. This role serves as a liaison between Clinical Operations, Information Technology, Quality Improvement, Compliance, and PACE leadership to ensure information systems effectively support participant-centered care. The position manages EHR optimization, workflow redesign, regulatory reporting support, end-user training, organizational change management, and application support across all PACE service lines, including primary care, nursing, rehabilitation, social services, home care, transportation, and center operations. The EMR Application Analyst & Trainer promotes efficient care coordination, regulatory compliance, data integrity, staff competency, and the successful adoption of technology that advances the PACE model of care., Clinical Information & Workflow Optimization * Partner with interdisciplinary team (IDT) leadership to evaluate and optimize workflows supporting participant care delivery. * Analyze clinical, operational, and regulatory processes to identify opportunities for workflow redesign and system optimization. * Design, document, and implement workflows that support coordinated care across medical, behavioral health, rehabilitation, social services, transportation, and home care programs. * Collaborate with Quality, Compliance, and Operations leadership to ensure workflows align with CMS, state, and other regulatory requirements. * Evaluate workflow effectiveness and recommend enhancements that improve participant outcomes, operational efficiency, and staff productivity. * Support development and maintenance of standardized clinical documentation practices. * Ensure configuration and workflow changes are documented, tested, and communicated appropriately. Program & Project Management * Lead technology and operational initiatives supporting PACE strategic priorities, including EHR implementations, upgrades, system enhancements, and optimization projects. * Develop and manage project plans including scope, timelines, milestones, resources, risks, and communication plans. * Facilitate organizational change management activities including stakeholder engagement, communication planning, training, and adoption strategies. * Monitor project performance and provide regular updates to executive leadership and operational stakeholders. * Coordinate with vendors, consultants, and third-party partners to support project objectives and operational requirements. Data Analytics & Regulatory Support * Develop reports, dashboards, and performance metrics that support operational decision-making and quality improvement. * Assist leadership with regulatory reporting requirements and data validation activities. * Monitor data quality and integrity across clinical and operational systems. * Analyze utilization, productivity, participant outcomes, and operational performance to support quality improvement initiatives. * Collaborate with Quality Improvement teams to identify trends and opportunities for improvement. Training & Workforce Development * Design and deliver training programs supporting clinical systems, operational applications, and regulatory workflows. * Develop curricula, training materials, job aids, workflow guides, competency assessments, and other learning resources. * Facilitate onboarding and ongoing education for interdisciplinary staff, providers, and operational teams. * Develop long-term learning strategies to support workforce competency and technology adoption. * Coordinate training schedules, classroom logistics, virtual learning activities, and go-live support. * Evaluate training effectiveness and implement continuous improvement initiatives. Application Support & User Adoption * Serve as a subject matter expert for clinical and operational software applications. * Provide advanced application support by troubleshooting end-user issues and resolving Help Desk tickets. * Support testing, validation, and deployment of new software releases and system enhancements. * Identify user adoption barriers and implement solutions to improve system utilization and efficiency.
