They excel at building relationships, navigating complex systems, solving problems, and translating ideas into meaningful programs and services that improve outcomes for the communities we serve., * Experience working within housing, healthcare, Medicaid-funded programs, or other human services systems. * Knowledge of the Homeless Management Information System (HMIS) and experience using data to evaluate programs, measure outcomes, and support reporting. * An understanding of Oregon Medicaid, Coordinated Care Organizations (CCOs), Health-Related Social Needs (HRSN), and Long-Term Services and Supports (LTSS). * Experience researching, evaluating, and improving programs through policy analysis, program evaluation, and performance measurement. * Strong contract management and program oversight skills, including monitoring performance, ensuring contract compliance, and partnering with providers to improve service delivery. * Excellent communication, collaboration, and relationship-building skills, with the ability to work effectively across government agencies, healthcare organizations, community-based organizations, and regional partners., * A minimum of four (4) years of related experience that would provide the required knowledge and skills to perform the responsibilities of this position * Demonstrated experience advancing racial equity in a workplace and/or community setting * Experience with the development and implementation of strategic plans and/or programs related to housing or healthcare * Knowledge of principles, methods, and procedures of local, regional and statewide planning * Ability to facilitate meetings and provide trainings with county and community partners * Experience analyzing and interpreting data using Microsoft Excel to evaluate program performance, identify trends, and support data-informed decision-making. As the Health and Housing Systems Integration Program Planner, you will play a key role in designing, implementing, and improving programs that connect individuals experiencing housing instability with critical healthcare and supportive services. Reporting to the Health and Housing Systems Integration Program Supervisor, you will lead the planning, coordination, and evaluation of programs and partnerships that bridge homeless services, healthcare, and other systems of care. This includes strengthening connections between Permanent Supportive Housing (PSH) programs and physical and behavioral healthcare, supporting the implementation of a new Medical Respite program, and coordinating Health-Related Social Needs (HRSN) services across internal departments and external partners. In this role, you will evaluate programs, identify opportunities to improve system coordination, and develop strategies that enhance access to integrated, person-centered services.
