Physical Requirements: * Ability to lift and carry approximately 25-75 pounds as required by individual support needs. * Capability to assist individuals with mobility challenges and other activities of daily living. * Stamina to perform various physical tasks and community-based activities., * Bachelor's (Required) Experience: * working with individuals with intellectual disabiltiies: 1 year (Preferred) ## Description The Case Manager & Direct Support Specialist provides both case management coordination and direct, person-centered support to individuals with intellectual and developmental disabilities. The position assists the QIDP and Division Director with coordinating, monitoring, documenting, and implementing services while also providing direct support to individuals in their homes and communities. The role promotes independence, choice, community inclusion, health, safety, and overall quality of life while helping ensure services are delivered in accordance with each individual's person-centered plan and applicable organizational and regulatory requirements., * Provide direct care and support to individuals with disabilities in their homes, workplaces, and community settings. * Assist with daily living activities, including personal hygiene, meal preparation, household chores, appointments, recreation, and community participation. * Partner with individuals to support informed decisions and everyday choices related to finances, well-being, relationships, employment, and personal goals. * Promote independence, self-direction, community integration, meaningful relationships, and skill development.
