Informed by statewide data and predictive analytics, community-based care-managers work with local physicians and diverse teams of health professionals to develop whole-person plans of care that connect people to the right local resources and increase equity and access to high quality care., As a Data Scientist at CCNC, you will build and maintain predictive models that support our proprietary Impactability framework, which identifies Medicaid patients most likely to benefit from care management outreach. You will analyze large claims, clinical, and social determinants of health datasets to surface trends, disparities, and opportunities for population health improvement. You will collaborate closely with clinical, care management, and network teams to translate analytic findings into practical program recommendations. You will also contribute to the continued development of CCNC's cloud-based analytics infrastructure on AWS, design and run retrospective and prospective program evaluations, and prepare clear data visualizations and reports for both technical and non-technical audiences including executive leadership, payers, and state partners. Additionally, you will support quality improvement initiatives by developing performance metrics and benchmarking tools used by practices across the state.
