Duties include conducting transition readiness assessments, acting as liaison between pediatric and adult care teams as needed, documenting all assessment forms, and effectively communicating and navigating systems to assure patient access. The division currently provides care for patients living with blood disorders, sickle cell disease, and oncology related disorders.
