Starting at $44,676 - $47,650; contingent upon experience, education, etc. This position will provide addiction intervention, AODA counseling and recovery support services to pregnant and post-partum mothers and their families. This position is responsible for bringing services, agencies, resources, and other people together to assist clients in achieving established goals. This position may also work with high-risk caseloads, co-occurring clients, families, and children. * Assist individuals in achieving service plan goals through direct services and coordination of behavior health, public agencies, and community resources. * Assess, evaluate, facilitate, and monitor individuals' progress with contractual benchmarks * Complete intake packets for program enrollment and participation * Provides referrals and crisis intervention for the individuals' identified needs upon release from prison * Interview individuals, review records, and confer with other professionals to identify reentry needs * Collaborate with internal and external partners, participate in staff meetings and monthly supervisions, and participate in integrated staffing with individuals receiving TASC services * Identify and refer individuals to initial treatment and behavioral health services * Complete and submit internal and external program reports * Complete and maintain all required individual and program related documentation * Assist clients with the enrollment in public benefit systems and obtaining vital records * Participate in community meetings, trainings and projects including multi-disciplinary staffing, vocational-and educational-programming, institutional counseling, correctional-industries, employers, and other meetings assigned * Develops effective working relationships with the internal and external partners * Participate in meetings to adjust, update, or revise individual service plan * Obtain and clinically analyze progress reports from internal and external partners to ensure * individuals' progress is on target with the benchmarks outlined in their service plan * Ensure that confidential information relating to the organization, its staff, and individuals is kept confidential * Be an excellent steward of TASC, modeling the core values of the organization ## Related Videos - [Maturity assessment for technicians or how I learned to love OWASP SAMM](https://www.wearedevelopers.com/videos/351-maturity-assessment-for-technicians-or-how-i-learned-to-love-owasp-samm) - [Agile work at CARIAD – Creating a customer web application for controlling the vehicle ](https://www.wearedevelopers.com/videos/200-agile-work-at-cariad-creating-a-customer-web-application-for-controlling-the-vehicle) - [Grappling With Clunky Old Software?
