You must have some understanding of health services together with a passion for improving lives and improving healthcare. ## Description This role leads digital transformation and datadriven improvement across Sphere PCN. The post holder will optimise digital tools, improve EMIS and general practice workflows, strengthen data governance, and support practices to deliver measurable improvements in patient care and operational efficiency. Working closely with clinical and operational teams, the post holder will develop a shared vision for digital and service transformation aligned with ICB and INT strategy. They will identify challenges, agree priorities, and support practices to implement sustainable change. Strong relationships with member practices, HNP, INT and ICB will be essential to ensure collaborative working, shared learning, and consistent delivery of improvements across the PCN. Main duties of the job Service Improvement & Transformation:Work with practices to understand operational challenges and identify practical solutions, including sharing best practice across the PCN. Lead the planning, prioritisation and delivery of KPIs; including changes that improve patient access, staff experience and service efficiency. Develop and maintain a PCN wide transformation plan, managing interdependencies, risks and resource requirements to deliver KPIs. Support staff to access relevant training and development to enable successful delivery of agreed improvements. Key Performance Indicators (KPIs): Increased uptake of NHS App and online consultation tools. Improved EMIS data quality and coding consistency. Measurable improvements in patient access and digital experience. Demonstrable reduction in unwarranted variation across practices. Develop PCN-led transformationProject Plans includingSOPs and SLAs. Information & Data Management:Highlight unwarranted variation and support practices to understand data limitations. Encourage culture of using data to evaluate operational processes and track trends. Secure analytical support where required and strengthen PCN data sharing mechanisms. Communication & Engagement:Build strong working relationships with practices andSupport consistent communication and engagement across the PCN. Ensure changes align with clinical priorities and have ownership from clinical leaders. Support PCN and Practices to implement new processes effectively to patients.
