## About the Role

* Master's Degree in Social Work and licensure in the State of Minnesota required (LGSW).
 * Experience in healthcare setting preferred
 * Experience using electronic health records preferred
 * Proficient in Microsoft Office Products including Word and Outlook
 * Able to effectively and efficiently communicate in written and verbal form
 * Ability to smoothly and rapidly shift between dissimilar tasks
 * Ability to communicate well both verbally and in writing
 * Ability to react quickly and calmly as part of a team to handle volatile and potentially emergency situations
 * Ability to make effective decisions while in stressful situations and circumstances
 * After training, be able to manage disruptive and explosive patients.

## Description

* Responsible for effective case psychosocial assessment and suicide risk assessment, discharge planning, and leading psychoeducational and/or therapeutic groups
 * Responsible to develop treatment plans, complete discharge plans and conduct discharge meetings
 * Responsible for communicating with patients and families to keep them involved in treatment plan decisions
 * Coordinates care with outpatient providers upon admission and discharge
 * Responsible for facilitating weekly treatment team meetings and creating master treatment plans/updating treatment plans as agreed upon by the treatment team
 * Contributes clinical perspective to the interdisciplinary team based on unique training in the area of family systems and/or social work theory.
 * Communicate clearly and effectively with clinical team members and Directors
 * Is oriented and educated in gender, cultural differences, and age appropriate care for the assigned department
