## Description

* The Platform Coordinator will need to have organizational skills with the ability to effectively prioritize and complete tasks to support Care Coordinators with platform referrals.
 * Coordinates to process patient referrals from Navi, Allscripts, Epic NCH, and Power chart, as well as other referral platforms.
 * Must be detail-oriented, possess the ability to multitask, and be open to cross-functional training in all departments.
 * Displays a high level of patient focus that works directly with Care Coordinators, Case Managers, Physicians/PCP(s), insurance representatives, outside vendors, and other medical personnel.
 * Pulls referrals off platforms to accept/decline, Reviews documents, confirms, and processes the referral through Intake using Pinnacle's EMR.
 * Responds to Care Coordinator inquiries regarding referral status promptly.
 * Responds to all inquiries with a resolution to concerns and or discrepancies.
 * Coordinates and communicates with the appropriate team regarding any changes or updates to the patient's payer source on time.
 * Meet productivity goals as assigned by the direct supervisor.
 * Complies with HIPAA regulations on all accounts.
 * All other responsibilities assigned by leadership.
