Licensure / Certification / Registration: * Required Credential(s):

 * Credentialed Alcoholism and Substance Abuse Counselor (CASAC), issued by the New York State Office of Addiction Services and Supports (OASAS), must be obtained prior to hire or job transfer date.

## Description

* Assists patients with activities of daily living and provides or supports direct patient care, diagnostic procedures, and therapeutic interventions.
 * Observes and documents patient behaviors, progress, and changes in condition; communicates findings to the healthcare team and contributes suggestions for treatment plan modifications.
 * Establishes and maintains therapeutic relationships with patients to support engagement in care.
 * Provides counseling services and case management, including assessment and treatment of substance use disorders using evidence-based tools and strategies, as permitted under CASAC guidelines.
 * Maintains accurate and timely documentation in accordance with reporting and recordkeeping standards.
 * Assists in care coordination, including discharge planning with the interdisciplinary team to support timely, efficient, and patient-centered outcomes.
