A central focus of this role is transitioning our current AI and speech-to-text (STT) pipeline to an event-driven architecture - designing the events, messaging, and streaming patterns that let us process clinical audio and AI workloads reliably at scale. You'll work in a fast, hands-on environment, partnering closely with founders and the engineering team to solve real clinical workflow problems in production., * Lead the transition of our AI and STT pipeline to an event-driven architecture: design the events, queues, and streaming flows and drive the migration end to end. * Partner with founders on backend strategy, architectural direction, and the technical roadmap. * Architect reliable, secure, scalable backend systems that handle clinical audio, AI workloads, and sensitive data in production. * Define messaging, queueing, and orchestration patterns (pub/sub, event streaming) and the standards for how services communicate. * Build observability, monitoring, and feedback loops to catch reliability issues and regressions before customers do. * Make pragmatic choices on frameworks, infrastructure, and tooling; balance reliability, cost, latency, and developer velocity. * Contribute across the stack as needed to ship features end to end (we're early; everyone wears multiple hats). * Establish backend engineering standards and help hire and mentor the team. * Ship quickly with customers; measure impact and refine. * Hold a high quality bar while executing autonomously. Our tech stack * Turborepo (monorepo) * Frontend + Core app: Expo/React Native (Web, Android, iOS) + Admin portal: Next.js * Backend: NestJS * Database: PostgreSQL * Infrastructure: GCP * AI: LangChain leveraging Anthropic, OpenAI, and Gemini models ## Related Videos - [Nest.js - TypeScript in the backend can also be clean](https://www.wearedevelopers.com/videos/1033-nest-js-typescript-in-the-backend-can-also-be-clean) - [Meet Your New BFF: Backend to Frontend without the Duct Tape](https://www.wearedevelopers.com/videos/682-meet-your-new-bff-backend-to-frontend-without-the-duct-tape) - [Developing the Backend with Stefan Lingler, CTO at Shpock](https://www.wearedevelopers.com/videos/100360-developing-the-backend-with-stefan-lingler-cto-at-shpock) - [From DevOps to Scaled DevOps: How We’re Rebuilding Continuous Delivery as a Platform](https://www.wearedevelopers.com/videos/100018-from-devops-to-scaled-devops-how-we-re-rebuilding-continuous-delivery-as-a-platform) - [Next, Nest, Nuxt… Nust?](https://www.wearedevelopers.com/videos/421-next-nest-nuxt-nust) - [Streaming AI Responses in Real-Time with SSE in Next.js & NestJS](https://www.wearedevelopers.com/videos/1630-streaming-ai-responses-in-real-time-with-sse-in-next-js-nestjs) ## Related Articles - [Dev Digest 120 - Apple and peers](https://www.wearedevelopers.com/magazine/455-dev-digest-120-apple-and-peers) - [Dev Digest 121 - AI goes offline](https://www.wearedevelopers.com/magazine/456-dev-digest-121-ai-goes-offline) - [What’s the Difference Between Frontend and Backend Development?](https://www.wearedevelopers.com/magazine/240-what-s-the-difference-between-frontend-and-backend-development) - [Highest Paying Tech Companies for Developers](https://www.wearedevelopers.com/magazine/220-highest-paying-tech-companies-for-developers) - [Dev Digest 132 - Binging WADFlix?](https://www.wearedevelopers.com/magazine/473-dev-digest-132-binging-wadflix) - [Find a Developer Job: 12 Best Job Sites For Developers](https://www.wearedevelopers.com/magazine/165-find-a-developer-job-12-best-job-sites-for-developers)
