 * Assist clients in accessing necessary resources and services to meet their needs.
 * Offer information, referral services, brief counseling, or extended case management to clients.
 * Collaborate with team members to deliver comprehensive counseling and case management services.
 * Develop and maintain partnerships with internal and external service providers.
 * Conduct assessments to identify and address the needs of clients seeking services.
 * Accurately enter client information into relevant internal databases.
