Three to five years of experience or training beyond a bachelor's degree in working with persons with disabilities providing transition training, technical assistance and/or direct services. Knowledge, Skills & Abilities * Knowledge of transition to adulthood services and supports, * Knowledge of disability services and supports, * Skill in managing one's own time and the time of others, * Ability to analyze, organize and prioritize work while meeting multiple deadlines, * Ability to analyze and prepare documents, reports, and correspondence, * Ability to process and handle confidential information with discretion, * Ability to communicate verbally and through written means to interact with a diverse group of people, * Skill in being detail-orientated and timely with all required notes, files and documentation, * Ability to establish goals and objectives, and * Ability to be resourceful and determined to provide a positive experience for others. Preferred Qualifications Master's degree from an accredited college or university in disability studies, social work, healthcare, special education, psychology, child and family sciences, counseling or related field. Social work licensure or case management certification. ## Description The Transition Coordinator coordinates day-to-day operations of transition to adulthood (employment, volunteerism, service, independent living, leadership, advocacy, and post-secondary education) programs and services.
