Job description

The Transition Coordinator provides leadership and coordination of districtwide transition services for students with disabilities. The role ensures compliance with federal and state transition requirements, supports school teams in developing high-quality transition plans, oversees work-based learning and community-based instruction initiatives, manages transition-related grants, and collaborates with families, agencies, and district staff to promote successful post-school outcomes for students. Essential Functions Essential functions, as defined under the Americans with Disabilities Act, may include, but are not limited to, the following tasks, duties, knowledge, skills and other characteristics. ESSENTIAL DUTIES AND RESPONSIBILITIES generally include the following. Other duties may be assigned:

  • Coordinates and oversees transition services, programs, and supports across the district.
  • Provides coaching, guidance, and technical assistance to school staff regarding transition assessments, transition goals, and Indicator 13 compliance.
  • Develops and implements districtwide transition initiatives and services, including course development, instructional resources, work-based learning, community-based instruction, and career and technical education opportunities, and postsecondary readiness.
  • Manages transition-related grants, including planning, budgeting, documentation, compliance, and reporting requirements.
  • Maintains up-to-date knowledge of all legal requirements and best practices concerning IDEA transition services and postsecondary transition planning.
  • Assumes responsibility for compiling, maintaining, and filing all reports, records, and documentation required for state and federal compliance, including Indicators 13 and 14.
  • Coordinate and collaborate with other district departments in support of general education, special education, transition, and college and career initiatives including School Counseling, Career & Technical Education, Learning & Instruction, Business Services, and Human Resources.
  • Coordinates interagency collaboration with Vocational Rehabilitation, Division of Developmental Disability behavioral health, DES, employment partners, colleges, and community organizations.
  • Develops and provides training opportunities related to transition planning, employment readiness, self-advocacy, and student-led IEP participation.
  • Supports the coordination of transportation needs related to transition activities, work-based learning, and community-based instruction.
  • Serves as a resource for parents, principals, case managers, and teachers regarding transition services and postsecondary pathways.
  • Works cooperatively with other administrative departments to ensure aligned services for students with IEP transition plans or Section 504 plans where appropriate.
  • Collaborates with district staff to analyze transition data and use it to guide program development and improvement.
  • Performs other duties as assigned or required.

Requirements

Do you have a valid Driver's License license?, Do you have experience in Research?, + Knowledge of applicable Federal, state, county, and city statutes, rules, policies, and procedures.

  • Knowledge of applicable district and departmental policies and procedures.

  • Thorough understanding of IDEA transition requirements and the Rehabilitation Act of 1973 (Section 504).

  • Knowledge of transition assessments, work-based learning models, and postsecondary disability services.

  • Understanding educational research, career development frameworks, and transition-related data analysis.

  • Ability to design, deliver, and evaluate professional development.

  • Excellent written and verbal communication skills.

  • Ability to apply planning strategies for districtwide transition services.

  • Strong interpersonal skills and the ability to collaborate with diverse teams, families, and community partners.

  • Integrity, professionalism, and a commitment to equitable post-school outcomes for students with disabilities.

  • Skill in managing and supporting staff and coordinating program initiatives.

  • Skill in establishing and maintaining effective working relationships with coworkers, students, parents, agency representatives, and the general public.

  • Skill in operating a personal computer utilizing a variety of software applications. MINIMUM QUALIFICATIONS

  • Degree in Special Education or related field such as Occupational or Physical Therapy, Speech Language, and/or Social Work.

  • Three years of teaching experience or experience working directly with transition-age students.

  • Must hold a professional license or certificate related to a professional degree.

  • Experience supporting transition services or program coordination preferred OR an equivalent combination of experience and/or education from which comparable knowledge, skills, and abilities has been achieved.

  • Grant management experience preferred (or willingness to be trained). OTHER QUALIFICATIONS

  • Must possess and maintain a valid Arizona IVP Fingerprint Clearance Card.

  • Must be able to pass a background check.

  • May be required to work outside normal working hours.

  • Must have and maintain a valid Arizona driver's license.

  • May be required to travel to perform job functions.
