Job description

  • Provide sexual health education on HIV, STIs, HCV, PrEP, and PEP
  • Connect individuals to HIV/STI/HCV testing and prevention services
  • Serve as a Patient Navigator and Health Educator, helping patients understand and access healthcare services
  • Support patients with care navigation, appointment scheduling, and follow-up
  • Conduct community outreach and educational presentations
  • Assess social determinants of health and connect patients to support services
  • Coordinate care with healthcare providers and program staff
  • Maintain accurate documentation and program data in the electronic medical record system

Requirements

Do you have a valid Driver's License license?, Do you have experience in Teamwork?, Do you have a High school diploma or GED?, We are seeking a Spanish-Bilingual HIV/STI Prevention Specialist who is passionate about community health and committed to expanding access to prevention services.

In this role, you will help individuals access HIV/STI/HCV education, testing, and prevention resources, while supporting patients in overcoming barriers to care and improving their overall health. This position is ideal for someone who enjoys community outreach, health education, and helping underserved populations access care.

No prior HIV prevention experience is required, training will be provided. Candidates with experience as a Patient Navigator or Health Educator are encouraged to apply., * Associate's degree accepted

  • Bachelor's degree (BA/BS) or coursework at this level preferred

Preferred Experience

  • Experience working in healthcare, public health, or social services settings
  • Experience with electronic medical records or data tracking/management

Requirements

  • Valid New York State driver's license
  • Daily access to a reliable vehicle for community outreach, * High school or equivalent (Preferred)

Experience:

  • HIV/AIDS care: 2 years (Preferred)
  • Care plans: 2 years (Preferred)

Language:

  • Bilingual English/Spanish fluently (Required)
