Lifetime Independence for Everyone, Inc. (LIFE Inc.), an advocacy organization with innovative services and programs designed by and for people with disabilities, is searching for a highly motivated individual to join our Disability Connections' team in San Angelo, Texas! We are seeking to hire an Independent Living (IL) Specialist, who will be responsible for facilitating the delivery of Independent Living Services to a cross-disability population. The IL Specialist will determine program eligibility and arrange and/or provide goods and services that will enable Consumers to function independently in the home, family, and/or community. Starting salary range is $38,000 - $42,000, depending on experience. Fringe benefits include paid sick and vacation leave, retirement plan, generous holiday schedule and medical insurance.
Essential Tasks:
1.Understands and subscribes to the independent living philosophy.2.Conducts intakes and determines program eligibility.3.Works collaboratively with Consumers to develop an Independent Living Plan (ILP) designed to achieve the Consumers' identified goals.4.Provides direct services in the five (5) core areas of advocacy, information and referral, peer support, transition and independent living skills training.5.Maintains full, accurate, and current documentation of all information required in each Consumer's service record.6.Provides Consumers with one-on-one IL training as appropriate.7.Conducts follow-up calls and/or home visits with Consumers to assess progress toward goals, including the improvement of the Consumer's functional abilities as a result of services provided.8.Participates in meetings, in-service training, workshops, etc. for the purpose of conveying and/or gathering information required to perform relevant job functions.9.Maintains a thorough knowledge of appropriate health and social service delivery systems.
Requirements

Thorough knowledge of issues and challenges experienced by individuals with disabilities
Thorough knowledge of social service delivery systems
Excellent organizational and time management skills
Ability to communicate effectively, both orally and in writing
Ability to maintain a positive attitude and work well with individuals from a variety of backgrounds
Ability to maintain a professional attitude, particularly in stressful situations
Attention to detail and discretion with confidential information
Knowledge and competence of computers and software applications, particularly Microsoft Office
Ability to exercise good judgment and take initiative
Education/Experience:
Bachelor's Degree in the field of Human Services, with a minimum of two (2) years' experience in working with individuals with disabilities and assisting them in navigating various service delivery systems to obtain essential resources; or, any combination of education and experience that may qualify the individual for the position.
