Job description
As a Care Transition Manager, you will build and maintain referral partnerships, guide families through senior care placement decisions, and provide hands-on support throughout the transition process. This role combines community outreach, client advocacy, and ongoing case management.
This position will primarily serve the Mount Pleasant, Alma, and Greenville areas, with occasional responsibilities in Big Rapids, Grand Rapids, and Lansing., * Identify and cultivate referral partnerships with hospitals, rehabilitation facilities, physicians' offices, senior communities, home health agencies, hospice organizations, and other related professionals.
- Meet regularly with social workers, discharge planners, and healthcare professionals who assist seniors transitioning from home or healthcare settings.
- Attend networking events and community functions to build reciprocal referral relationships and identify valuable resources for clients and families.
- Represent CarePatrol professionally within the community while promoting our mission and services.
Client Support & Advising
- Conduct in-depth care discovery meetings with seniors and their families.
- Assess client needs and provide guidance regarding appropriate care options.
- Schedule and conduct tours of senior living communities and care homes.
- Support families throughout the placement process, helping minimize stress and confusion.
- Maintain consistent follow-up communication with families and referral partners to ensure a smooth transition and positive outcomes.
Administrative & Team Participation
- Accurately document referrals, client interactions, and outcomes within the company database.
- Utilize task management systems to ensure timely follow-up and completion of responsibilities.
- Participate in weekly team meetings and individual one-on-one meetings.
- Perform additional duties and special projects as mutually agreed upon.
Requirements
Do you have experience in Time management?, We are seeking a dedicated, compassionate, and relationship-driven professional to join our growing team. A clinical background is a plus, and experience in marketing, networking, and relationship building is strongly preferred., * Minimum of one year of experience working with seniors or within the senior care industry.
- Knowledge of Medicaid Waiver programs and VA Aid & Attendance benefits.
- Familiarity with senior living communities and adult foster care homes within the Western/ Mid-Michigan territory.
- Experience in marketing, networking, business development, or relationship management.
- Clinical background preferred but not required.
