Job description

Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas. Meals on Wheels Midland Care's Meals on Wheels program in Topeka, Kansas, is a vital service that delivers hot, nutritious meals and wellness-checks to homebound seniors and individuals with disabilities in Shawnee, Jefferson, and Douglas counties. The program prioritizes individuals at high nutritional risk and is not income-based, allowing anyone who meets the age or homebound criteria to participate. With the help of hundreds of volunteers who deliver meals Monday-Friday, Midland Care supports independence, reduces social isolation, and ensures older adults receive balanced meals in the comfort of their homes. Program Assessor The Program Assessor for Meals on Wheels is responsible for evaluating and certifying clients for home-delivered meal services, ensuring they meet program eligibility requirements and receive appropriate support. This role conducts initial and ongoing client assessments, maintains accurate documentation and electronic records, coordinates re-certifications, and helps connect clients with additional community resources when needed. The Program Assessor also monitors service quality, responds to client concerns, supports efficient meal delivery operations, and collaborates with internal teams and community partners to ensure clients receive compassionate, high-quality care while maintaining compliance with program regulations and organizational standards.

Requirements

Do you have a valid Driver's License license?, Do you have experience in Referral coordination?, * Valid driver's license, reliable transportation, and the ability to travel throughout the service area to conduct in-home assessments

  • Experience conducting client assessments, interviews, or case management
  • Strong organizational skills with the ability to maintain accurate electronic documentation and client records
  • Excellent communication and interpersonal skills with the ability to build rapport with older adults and their families
  • Knowledge of community resources and the ability to make appropriate referrals
  • Ability to manage multiple priorities while working independently
  • Basic computer proficiency, including electronic health records and Microsoft Office applications
  • Commitment to maintaining client confidentiality and adhering to ethical and regulatory standards, * Previous experience working with older adults, Meals on Wheels, aging services, home and community-based services, or case management
  • Experience completing eligibility assessments for government or community-based programs
  • Knowledge of Medicare, Medicaid, Older Americans Act programs, or other aging-related services
  • Familiarity with electronic health record (EHR) systems
  • Experience with crisis intervention and coordinating community resource
  • Experience with quality improvement initiatives, reporting, and program compliance
