Job description

Respond to direct inquiries for HCBS or CHOICES program information with pertinent and accurate information which may include detailed information about the application and enrollment progress, program eligibility and enrollment criteria, and benefits.

Conduct the initial screening.

Provide accurate, consistent, comprehensive information in order to avoid the need for multiple referrals, screenings, and assessments.

Assess the short-term needs of an inquirer who is in crisis.

Provide comprehensive information and referral services on available long term care resources to consumers and professionals.

Respond to all requests for I & R received by fax, e-mail, or telephone within two business days, except during high volume call periods.

Mail a letter and information regarding the HCBS and CHOICES program to all callers with an available address after three unsuccessful attempts to contact via telephone.

Utilize standardized forms and/or databases to collect I & A data.

Participate in training as provided by TCAD / TennCare / AAAD.

Represent the agency in a professional manner at all times.

Abide by FTDD Personnel Rules and Regulations as may be amended from time to time..

Requirements
Ability to utilize listening skills, problem solving and referral techniques in order to access a broad spectrum of community resources. Ability to communicate effectively, both orally and in writing. Ability to be flexible to address unanticipated situations in a professional manner. Proficiency in usage of personal computers and appropriate software. Knowledge of community resources is required., Minimum of completion of grade 12. Prefer at least 2 years college and minimum of 2 years employment in field of social work and working experience in a professional office environment
