Job description
To receive and process requests for medical record information in accordance with Federal and State Statute and Hospital Policy, while protecting patient confidentiality and right to access., 1. Monitor and control the timeliness of outstanding requests for medical information. Receive and process requests for medical record information in accordance with Federal and State statute and Hospital policy, ensuring that all documentation is obtained and submitted in accordance with departmental procedures.
- Validates that requestor has the authority to obtain medical record information in accordance with Federal and State Statute, third party contracts and/or Hospital Policy.
- Reviews, retrieves and prepares medical record documentation in response to requests; contacts staff at other CCHC sites who maintain decentralized medical record files as well as off-site storage vendors to obtain medical record information.
- Communicates and interacts with customers who request medical record documentation, ensuring requests are responded to appropriately and in a timely manner. As needed, provides instructions to requestors related to procedures for obtaining medical record information and patient consent requirements.
- Tracks and logs requests for medical record documentation in accordance with departmental procedures, trending and interpreting patterns and variances.
- Performs all duties and interacts with others in accordance with the Hospital's Customer Service standards.
- Performs other work related duties as assigned. Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers in a manner that reflects Cape Cod Healthcare's commitment to CARES: compassion, accountability, respect, excellence and service.
Requirements
Ability to read, write and communicate in English.
- High School Diploma or GED.
- Two years of current experience in a medical setting
- Excellent written, verbal communication and interpersonal skills.
- Ability to prioritize and manage multiple tasks simultaneously, and to effectively anticipate and respond to issues as needed in a dynamic work environment.
- Knowledge of Microsoft Word, Excel, and Outlook.
