POSITION SUMMARY: The Social Care Network, Support Specialist will work within the Social Care Network (SCN) Team in the Southern Tier region of New York and will be responsible for supporting the Network Performance Specialists in the oversight and provision of enhanced social care services. This position reports to the Network Operations Manager., The responsibilities of the Network Support Specialist will include the following, * Support the Network Performance Specialists (NPS) and SCN Operations Team in network management activities, including managing referral backlogs and monitoring network capacity to preserve continuity of care for Medicaid member services.
- Maintain high understanding of and demonstrated proficiency in SCN programmatic requirements, including care delivery workflows and documentation of services in Unite Us to support the seamless delivery of enhanced health related social needs (HRSN) services.
- Review, remediate and approve enhanced service referral requests in the domains of housing, nutrition and non-medical transportation to ensure network adherence to programmatic guidance and invoicing standards, supporting specific priority service domains as needed (ex. housing).
- Collaborate with network providers at the user level, including social care navigators and social care service providers to manage referrals for enhanced health related social need (HRSN) services, and provide technical assistance as needed.
- Work with internal cross-functional teams to meet partner needs and ensure positive partner experience.
- Monitors and reports on progress and results for current initiatives and ensure that they meet expectations and deadlines. Manages and communicates ongoing changes in tasks, goals and performance.
- May assume additional responsibilities, as required.
- MUST Maintain HIPAA compliance at all times.
Requirements
- Bachelor's degree and at least 2 years equivalent experience in navigation or care management role.
- Experience working with/within community-based organizations and social care service delivery in the domains of navigation/care management, housing, nutrition and/or transportation.
- Existing familiarity with housing social care services in the 6-county region is preferred but not required.
- Exceptional ability to build/maintain collaborative working relationships with internal and external stakeholders.
- Proficient in planning, time management and ability to prioritize many tasks in a constantly changing environment.
- Strong organizational and communication skills (written and verbal), including the ability to work independently with minimal supervision.
- Must have strong personal computer program skills including Microsoft Outlook, Microsoft Office (Word, Excel PowerPoint etc.)
- Facilitation of meetings either in person or via virtual platforms such as Zoom
