The Network Success Specialist 2 is primarily responsible for building and maintaining collaborative relationships with physician practices, post-acute providers, and other strategic partners to support value-based care initiatives. This role drives provider engagement through education, communication and performance-focused collaboration to support high-quality patient outcomes and organizational goals. Additionally, the Network Success Specialist 2 serves as a relational leader, facilitating provider education, strengthening alignment between physician practices and post-acute providers, and promoting best practices that support coordinated patient-centered care. This role supports ACO initiatives by improving provider communication, encouraging effective care transitions, and fostering engagement strategies that align with organizational objectives., * Build and maintain strong relationships with physician practices and post-acute providers to support value-based care initiatives.
- Educate physician practices and post-acute providers on care coordination strategies, organizational goals, and best practices related to post-acute utilization and patient outcomes.
- Facilitate collaborative discussions between providers to improve communication, care transitions, and alignment across the post-acute care continuum.
- Support Provider engagement efforts by conducting routine meetings and follow-up activities focused on quality improvement and coordination of care.
- Promote provider accountability and partnership through ongoing relationship management to support operational alignment and organizational goals.
- Assist with initiatives that encourage appropriate utilization patterns, improved patient outcomes and strategic engagement.
- Document and provide feedback to leadership team on all customer interactions in Salesforce.

- Conducts practice assessments and documents engagement tracking functions
- Identifies and provides solutions and interventions for providers whose key metrics are underperforming.
- Works with Provider Success Team to encourage physicians to utilize the services of high-quality vendors and Preferred Providers.
- Conducts In-Person meetings and virtual meetings where professional standards are upheld
- Handles office tasks, such as scheduling conference calls and meetings
- Maintains polite and professional communication via phone, e-mail, and mail
- Engages providers and educates them on performance improvement initiatives
- Provides Management with feedback from providers with issues and concerns
- Works closely with Provider Success Team, Contracting Team and other departments to maintain open lines of communication

Requirements
- Bachelor's degree in Business, Healthcare or related field.
- Possess 5+ years' experience in a client facing healthcare role or sales field., * Proficiency in Excel and KPI management, with proven ability to build data-driven reporting.
- Excellent communication and relationship-building skills across provider and executive audiences.
- Strong written, oral, and interpersonal communication skills.
- Strong initiative skills.
- Strong analytical and data management skills.
- Must be detail-oriented, possess exceptional communication and inter-personal skills, organization skills and a spirit with a willingness to go above and beyond the call of duty in order to complete projects accurately and timely.

Physical Requirements
This position works under usual office conditions. The associate is required to work at a personal computer as well as be on the phone for extended periods of time. Must be able to stand, sit, walk and occasionally climb. The incumbent must be able to work extended and flexible hours and weekends as needed. Physical demands include ability to lift up to 50 lbs.
