Case Manager (FT) (Bi-lingual Spanish)
The Opportunity
The Case Manager will deliver youth-focused gun violence prevention and community violence intervention programming for FCA's Empowerment Through Wellness to Prevent Gun Violence Initiative.
Rate: $27.47/ An Hour
Responsibilities

Work as an onsite case manager and member of the team to support youth charged with gun-related offenses.

Provide individualized case management for all participants.

Make appropriate cross-system referrals i

Life Skills: Harm reduction, parenting, healthy habits, financial literacy, and planning.

Manage, build and create relationships with local agencies and service providers.

Assist youth in navigating systems including providing court advocacy and transportation to medical appointments.

Work with the court staff/resource coordinators, mentors and therapeutic counselor and assist them in providing case management services to their high-risk program participants.

Coordinate case planning/ service delivery with assigned court staff, including Resource Coordinators, and make appropriate cross-system referrals.

Engage with other community-based service providers as appropriate to ensure a wraparound approach.

Be available to support the team in emergencies regarding incidents with staff or participants.

Adhere to all documentation and database requirements.
Requirements

Bachelors Degree in related field with appropriate experience.

Bi-lingual English-Spanish required.

2+ years of experience in working with at risk populations.

Proficient computer skills with MS Office

Valid and clean driver's license.
