Job description
The Cooper County Board of Sheltered Services (CCBSS) is seeking a full-time Support Coordinator (Case Manager) to provide case management services for individuals with developmental disabilities. This position is responsible for assessing needs, coordinating services, monitoring supports, maintaining documentation, and advocating for individuals to help them achieve their personal goals and maximize independence.
Support Coordinators work collaboratively with individuals served, families, providers, schools, healthcare professionals, and community agencies to ensure quality services and compliance with applicable regulations.
Essential Duties and Responsibilities
- Coordinate and monitor services for individuals with developmental disabilities.
- Conduct assessments and develop person-centered support plans.
- Maintain accurate, timely, and complete electronic documentation in accordance with agency, state, and federal requirements.
- Conduct home, school, provider, and community visits as needed.
- Monitor health, safety, and overall well-being of individuals receiving services.
- Coordinate with families, guardians, providers, medical professionals, schools, and other community partners.
- Assist individuals in accessing available community resources and benefits.
- Maintain confidentiality of protected health information and agency records.
- Participate in staff meetings, required trainings, and professional development.
- Perform other duties as assigned.
Requirements
- Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, Special Education, Counseling, or another related field required.
- Valid driver’s license and reliable transportation for required travel.
- Ability to organize work, manage multiple priorities, and meet deadlines.
- Strong written, verbal, and interpersonal communication skills.
- Basic computer proficiency, including Microsoft Office and electronic documentation systems.
Preferred Qualifications
- Experience providing case management or human services.
- Experience working with individuals with developmental disabilities.
- Knowledge of Missouri developmental disability services and Medicaid programs., The position requires the ability to sit, stand, walk, drive, communicate effectively, and use standard office equipment. Work Schedule
This is a full-time position with a typical Monday-Friday work schedule. Some local travel is required to conduct visits and attend meetings.
