Job description
We are looking for seasoned contractor architects to lead integration architecture for a healthcare digital platform connecting to external providers via custom REST APIs, FHIR-compliant endpoints, and complex authentication mechanisms. You will work alongside internal engineering teams to design secure, scalable, and compliant integration patterns - and bring a forward-looking perspective on AI-native and agentic architecture., * Design and own end-to-end integration architecture for external provider systems using custom APIs and FHIR R4/R5 standards.
- Define and implement authentication and authorization patterns including OAuth 2.0, SMART on FHIR, mTLS, API keys, and token exchange flows.
- Translate healthcare regulatory requirements (HIPAA, CMS Interoperability Rule, TEFCA, Da Vinci) into concrete architecture guardrails.=
- Evaluate and recommend API gateway, service mesh, and middleware patterns for health data exchange at scale.
- Architect agentic AI solutions - including LLM tool use, multi-agent orchestration, and RAG pipelines - within a healthcare compliance context.
- Partner with product, engineering, and security teams to produce Architecture Decision Records (ADRs) and integration blueprints.
- Conduct architecture reviews, identify risks, and define mitigation strategies across provider integration workstreams., Embedded with internal engineering squads - not a hands-off advisory role. Expected to produce artifacts: ADRs, integration blueprints, sequence diagrams, threat models. Expected to build POCs and demonstrate capability to senior leadership Collaborative with tech leads and principal engineers; able to mentor mid-level architects. Direct access to product and engineering leadership for key decisions.
Requirements
- Deep expertise in FHIR R4/R5 - resource modeling, RESTful APIs, bulk data operations, and CDS Hooks.
- Hands-on experience designing custom API integrations with external healthcare providers (payers, PBMs, EHRs, benefits platforms).
- Strong command of modern auth patterns: OAuth 2.0, OpenID Connect, SMART on FHIR, JWT, mTLS, and API gateway security.
- Healthcare regulatory fluency: HIPAA Privacy/Security Rules, CMS Interoperability and Patient Access rules, 21st Century Cures Act.
- Experience with agentic AI architecture - LLM orchestration frameworks (LangChain, LlamaIndex), tool/function calling, multi-agent patterns.
- Proven track record delivering integration solutions at enterprise scale in health plan, PBM, or provider organizations.
Nice to Have
- Experience with Da Vinci Implementation Guides (PDex, CDex, PAS, Prior Auth).
- Familiarity with TEFCA / QHINs and emerging health information network standards.
- Exposure to CVS Health, Caremark, Aetna, or similar health plan/PBM ecosystems.
- Knowledge of CMS IAL2 identity assurance requirements.
- Cloud architecture experience on Google Cloud Platform or Azure in a healthcare context (HIPAA BAA in place).
- Prior work with benefits enrollment platforms (bswift, Workday Benefits) or HSA/FSA custodians.
- CMS Medicare integration experience - Medicare Advantage APIs, Part D data exchange, EOB/Claims resources via Blue Button 2.0, and CMS-mandated Prior Authorization API (CMS-0057).
