Job description

We are seeking a motivated, organised, and compassionate PCN Care Co-ordinator to join our Primary Care Network team., This is an exciting opportunity for an experienced General Practice administrator who is passionate about improving patient care and supporting patients with long-term conditions, complex needs, and wider health and wellbeing requirements.

The successful candidate will work closely with GP practices, clinicians, pharmacists, social prescribing link workers, community services, and external organisations to provide coordinated, patient-centred support.

The role will focus on helping patients navigate healthcare services, coordinating appointments, supporting care plans, improving patient engagement, and helping prevent avoidable hospital admissions.

Key Responsibilities

  • Act as the first point of contact for a defined caseload of patients.
  • Coordinate patient appointments, reviews, follow-ups, and care pathways.
  • Support proactive call and recall processes for patients with long-term conditions.
  • Work closely with GPs, practice teams, pharmacists, social prescribing link workers, and community services.
  • Support personalised care planning and shared decision-making processes.
  • Assist patients in accessing appropriate health, social, voluntary, and community services.
  • Coordinate and support Multi-Disciplinary Team (MDT) meetings, including preparing agendas, taking notes, and following up actions.
  • Maintain accurate patient records, documentation, and coding on clinical systems.
  • Support administrative processes within the GP practice.
  • Use digital systems including MS Teams, NHS email, EMIS Web, and other healthcare platforms.
  • Identify barriers affecting patients and support solutions to improve access and outcomes.
  • Maintain confidentiality and comply with GDPR, information governance, safeguarding, and NHS policies.

Requirements

Previous GP Practice Experience is Mandatory

Applicants must have:

  • Previous experience working within a GP practice environment.
  • Strong working knowledge and practical experience using EMIS Web.
  • Experience managing patient records, clinical correspondence, coding, and administrative workflows.
  • Understanding of NHS primary care processes and patient pathways.
  • Excellent communication skills with the ability to communicate effectively with patients, clinicians, and external organisations.
  • Strong organisational skills with the ability to prioritise workload and meet deadlines.
  • Excellent attention to detail and accurate data entry skills.
  • Ability to work independently and as part of a wider healthcare team.

Desirable Experience

  • Experience in care coordination, personalised care, or patient navigation roles.
  • Experience supporting patients with long-term conditions.
  • Experience working with community services, voluntary organisations, or social prescribing teams.
  • Experience supporting MDT meetings.
  • Knowledge of Wandsworth health and community services.
  • Experience with NHS systems including Docman, NHS.net, and MS Teams., * Compassionate and patient-focused.
  • Proactive and solution-focused.
  • Highly organised and reliable.
  • Confident communicating with patients and healthcare professionals.
  • Able to manage sensitive information professionally.
  • Calm under pressure with the ability to manage competing priorities.
  • Committed to delivering high-quality patient care.

The successful candidate will receive appropriate induction and ongoing development opportunities, including relevant NHS training.
