Job description

  • Responds to issue resolution requests from clinicians, hospital departments, patients, andfamilies in a timely manner. Educates employees on reporting and correcting mistakes in health record documentation.
  • Keeps current on changes in regulations and policies affecting patient identity management,sharing with other departments. Prepares meeting materials and conducts meetings to educate staff, and collaborates with both intradepartmental teams and external department representatives on issues related to patient identity.
  • Researches and analyzes potential MPI duplicates and merges medical records according toprocedures. Disseminates merge and MRN changes to all downstream systems and departments. Determines patterns of redundant MRN creation and communicate findings to HIM management and other departments as necessary.
  • Maintains and updates demographic information in the MPI system to ensure the most currentand accurate identification of each patient. Monitors and evaluates patient demographic overlay changes to ensure integrity of patient information. Analyzes and reports trends in erroneous demographic changes.
  • Investigates reports of mis-identity or identity theft in close collaboration with the Privacy Office.
  • Identifies and research charting errors, including incorrect patient selections and rectify MPIcompromises using chart correction functions such as contact moves, unmerges, and documentation revision. Coordinates correction efforts with other affected departments by assigning and verifying completion of chart correction tasks. Audits historical medical records to ensure proper procedures were followed.
  • Coordinates with Registration, Billing, Social Work, and other appropriate departments to handlehe records of adopted children in a consistent manner that maintains patient family confidentiality and adheres to HIPAA regulations.
  • Works closely with IT to identify and resolve complicated documentation issues. Develops andexecutes reports, provides support and data to IT for systems improvement initiatives, and contributes to information quality and integration projects.
  • Manages proxy access to MyChart patient portal based on authentication of completed formsand provide documents for scanning.
  • According to rotating on call schedule, assists with high level identity issues that occur outside ofregularly scheduled shift.

The Health Information Identity and Chart Correction team is hiring a HIM Data Coordinator to investigate and resolve complex cases, helping our care team deliver the best possible care to our patients through accurate and reliable medical records.

This position is ideal for someone who is highly organized and detail-oriented, with excellent communication, problem-solving, and decision-making skills. An RHIA or RHIT credential is preferred, along with HIM experience. EPIC experience is a plus.

  • Detail oriented with analytical capabilities and deductive reasoning skills. Composure, tact andmaturity required in the interaction with physicians, coworkers and other customers.
  • Ability to work independently without direct supervision. Demonstrated problem-solving skills.Proficiency with Epic and Microsoft Office 365 preferred.
  • Knowledge of HIM operations (registration, documentation, and release of information).Understanding of the business impact of data integrity issues.
  • Demonstrated professionalism by developing loyalty, teamwork and respect with colleagues atall levels within the organization. Good facilitation and conflict resolution skills.
  • Aptitude for problem-solving involving complicated, interrelated and sometimes conflictingcomponents requiring careful observation, pattern identification and strict adherence to process sequencing.
  • Ability to communicate with diplomacy and professionalism in both writing and speech.
  • Strong ability to calmly manage simultaneous priorities, comfortable taking direction fromcolleagues at all levels, skillful working as a facilitator between multiple parties, able to maintain clear focus and work successfully under stressful conditions.
  • Capacity to produce quality work in both collaborative and independent tasks., * Associate’s degree in a computer related field, or equivalency.
  • Three years of experience working in a Health Information Department, or equivalency.
  • Extensive knowledge of health information processes and HIPAA and other regulatory agencies.
  • Course completion in Medical Terminology, Anatomy, Physiology, and Basic Coding.

Qualifications (Preferred)

  • Knowledge of EHR, document tracking systems, and/or billing/coding systems.
