Job description

Thriving Mind South Florida is seeking a dedicated Adult System of Care Specialist to support adults with serious mental illness involved in state hospital and forensic systems. In this critical role, you will coordinate services, facilitate successful transitions from institutional care to community-based treatment, and collaborate with hospitals, providers, courts, treatment teams, and other stakeholders to ensure continuity of care and positive outcomes.

The position reports to the Adult System of Care Manager.

ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following, as well as other work-related duties as assigned:

  • Serve as the liaison to the South Florida State Hospital.
  • Coordinate and monitor admissions to and discharges from state mental health treatment facilities.
  • Facilitate communication and collaboration among state hospitals, community providers, treatment teams, and system stakeholders.
  • Manage and monitor the civil commitment process, ensuring compliance with applicable laws, regulations, and timelines.
  • Assist with the coordination and tracking of individuals placed on the State Hospital Seeking Placement List.
  • Maintain and update tracking systems, including Smartsheet databases, to ensure accurate and timely reporting of admissions, discharges, placements, and service outcomes.
  • Provide oversight and monitoring of Florida Assertive Community Treatment (FACT) teams.
  • Review clinical assessments to determine appropriate treatment referral for placement into the Florida Assertive Community Treatment Teams (FACT).
  • Review performance measures, program outcomes, track and monitor client progress in treatment.
  • Coordinate Forensic placement and services.
  • Prepare, compile, and submit forensic and state hospital reports as required by funding agencies, governing bodies, and organizational leadership.
  • Monitor individuals transitioning from state hospitals and forensic settings to community-based services and supports.
  • Identify barriers to discharge and placement and work collaboratively with providers and stakeholders to develop solutions
  • Participate in multidisciplinary meetings, case reviews, and stakeholder workgroups related to adult behavioral health services.
  • Provide technical assistance and guidance to community providers regarding state hospital, forensic, and civil commitment processes.
  • Collect data, design reports, and interpret data/information for monthly, quarterly, and annual reports as necessary.
  • Monitor agencies’ contractual agreements to ensure compliance with contract mandated program goals and requirements.
  • Research and make recommendations concerning funding requirements for current and project programs.
  • Represent the Network at workshops, on task forces and at various planning meetings when requested.
  • Perform other related duties and special projects, including community-based and/or agency events as assigned.
  • Assist providers with balancing access, quality of care, and cost efficiency.
  • Provide/Coordinate and make recommendations for training and technical assistance for provider agencies.
  • Advise provider agencies on Evidence Based Practices (EBPs); and assist providers with implementation of those EBPs.
  • Assume other tasks as assigned by the Adult System of Care Manager.

Requirements

Do you have experience in Stakeholder relationship building?, Do you have a Master’s degree?, QUALIFICATION REQUIREMENT: Knowledge of the adult system of care. Knowledge of admission criteria and clinical assessment instruments.

EDUCATION AND/OR EXPERIENCE: Master’s Degree preferred; Bachelor’s degree with 2-year experience in related field.
