Job description

The Transition Service Coordinator provides service coordination to eligible individuals aged 16-22. Primary service oversight includes accessing community resources and Family Support, participating in school and transition team meetings and facilitation of the transition into adult services.

The Transition Service Coordinator provides person centered planning in collaboration with identified team members to ensure appropriate services are being provided at all stages of the transition timeline.

They carry a blended caseload of targeted and non-targeted; this includes a minimum of 30 billable individuals.

Daily Responsibilities

  • Ensure that our individuals have a safe and secure place in which to live;
  • Ensure community opportunities for community involvement and integration;
  • Assisting in finding and obtaining employment or other productive interests;
  • Identifying and implementing supportive solutions to obstacles that prevent them from living meaningful and productive lives in our community;
  • Identifying and overcome gaps in resources

Requirements

Do you have a valid Driver’s License license?, Do you have experience in Productivity software?, Do you have a Associate’s degree?, We are Growing and need several team members!!!   • Proficiency in Microsoft Office suite
  • Detailed, strong time management, planning and organizational abilities
  • Knowledge of school-based services, Guardianship, Community Care Waivers, and Medicaid Benefits is a plus
  • Reliable form of transportation and a valid driver’s license are required
