Job description

Digital Health and Care Wales (DHCW) is seeking a Senior Clinical Data Modeller to lead the development of operational standards for the National Target Architecture (NTA), with a particular focus on patient identity and the Person (Patient) Data Standard. This is a strategically important role, working at national level to shape how health and care data is structured, shared, and trusted across systems. You will provide technical and clinical leadership, ensuring that data models enable safe, interoperable, and person-centred care., You will lead the design, development and maintenance of clinical data models used across key digital applications in Wales, providing national leadership for standards that underpin the National Target Architecture (NTA), with a particular focus on patient identity and person-level data. This includes creating, adapting and assuring standards that support interoperable APIs. Working with policy makers, analysts, clinicians and developers, you will translate service needs into structured models that reflect best practice and national guidance. The role leads and contributes to national and UK wide collaboration, sharing work with standards groups, professional bodies and openEHR/FHIR communities. You'll provide expert advice, support decision making and help ensure consistent, high-quality person and patient data across multiple programmes., Own and develop data models relating to patient identity, person demographics, and identity linkage, supporting accurate, consistent identification across health and care settings.

  • Lead work on the Person / Patient Data Standard, including definition, governance, and adoption across programmes and systems.
  • Collaborate with clinical, technical, and policy stakeholders to ensure models are clinically meaningful, technically robust, and implementable.
  • Ensure alignment with national and international standards (e.g. HL7 FHIR, SNOMED CT, NHS data standards).
  • Provide expert advice on data modelling, information architecture, and interoperability to programmes and delivery teams.
  • Support assurance and governance processes, including impact assessments and design reviews.
  • Represent the organisation in national forums, working groups, and standards bodies as required.
