Job description

  • Support daily operations of Day Habilitation Services and assist in delivering person-centered programming.
  • Assist in supervising, training, scheduling, and supporting Day Habilitation staff.
  • Help develop, implement, and monitor individualized Day Hab plans and outcomes.
  • Coordinate community volunteer opportunities and meaningful integrated activities for individuals supported.
  • Collaborate with families, interdisciplinary teams, and community providers to ensure coordinated services.
  • Monitor service delivery at home base and community sites and coordinate transportation supports.
  • Complete and maintain required documentation, reports, and compliance records.
  • Promote a professional, inclusive, and person-centered environment for staff and individuals supported.

Requirements

Do you have experience in Decision making?, Do you have a High school diploma or GED?, * Bachelor's degree in a related field with one (1) year of experience supporting individuals with disabilities; OR

  • Associate degree with three (3) years of related experience; OR
  • High school diploma/equivalent with five (5) years of related experience, including supervisory experience.
  • Strong communication, leadership, and organizational skills.
  • Ability to interpret policies, service plans, and regulatory requirements.
  • Demonstrated problem-solving and decision-making abilities.
  • Must meet Lifetime Assistance Vehicle Operator Requirements.
  • Ability to meet physical requirements of the role, including lifting up to 50 pounds and maintaining SCIP-R certification if required.
