Job description

We are looking for a Social Worker to join our team within our youth Partial Hospitalization Program. As a Social Worker, you will collaborate closely with psychiatric medical providers and therapists on the patient's primary treatment team to ensure a cohesive and effective plan of care., * Assessment and Planning: Conduct effective assessments and suicide risk evaluations, develop treatment and discharge plans, and lead educational or therapeutic groups.

  • Patient and Family Communication: Maintain open lines of communication with patients and their families to ensure they are actively involved in treatment decisions.
  • Care Coordination: Coordinate care with outpatient providers during patient admissions and discharges to ensure continuity of care.
  • Team Facilitation: Facilitate weekly treatment team meetings, creating and updating master treatment plans based on team consensus.
  • Clinical Contribution: Bring a unique clinical perspective to the interdisciplinary team, drawing on training in family systems and social work theory

Requirements

Bachelors Degree in Social Work required, Master's Degree in Social Work highly preferred, with licensure in the State of Minnesota required.

Previous Experience:

  • Experience in mental health highly preferred
  • Experience using electronic health records preferred
  • Proficient in Microsoft Office Products including Word and Outlook
  • Proficient in Microsoft Office Products including Word, Outlook, and Teams.
  • Able to effectively and efficiently communicate in written and verbal form.
  • Ability to smoothly and rapidly shift between dissimilar tasks.
  • Ability to respond appropriately in times of mental health crisis.

Benefits & conditions

Compensation for this role ranges from $27-$35 per hour depending on licensure and experience.
