Job description

DESCRIPTION: Community Support Services is a clinical case management program serving individuals diagnosed with mental illness, who live independently in the community. Clients live in either Agency owned housing or their own residences. Case Managers work with the individuals on their personal goals in order to meet their needs and enhance their quality of life.   • Computer skills, * Provide in home and on-site services and support as per each individual's treatment plan; including provision of direct service to clients in areas such as motivational interviewing, money management skills, daily living skills, symptom improvement, psycho-education, housing support, job skills training, etc.
  • Evaluate skills and needs of each individual and make recommendations to treatment team
  • Learn the 8 dimensions of wellness and multiple interventions in order to provide quality Medicaid services
  • Utilize an electronic health record (EHR) system to write clear and concise progress notes to bill for Medicaid reimbursable services
  • Provide crisis intervention services, as necessary, and crises prevention plans in an effort to improve crises response
  • Pass all Agency and program specific trainings

Requirements

Do you have a valid Driver's License license?, Do you have experience in Driving?, Do you have a Bachelor's degree in human services?, Full-Time Raritan, NJ based Computer skills a must !

Must possess valid NJ/NY/PA driver's license In order to be eligible for employment, applicant must not have more than 5 points on their driving record and no DUIs within the last 3 years., * Bachelor's degree in human services related field required

  • Experience working with individuals who have mental illness
  • Experience working with electronic health record system and Medicaid reimbursement programs a plus
  • Ability to pass criminal/registry background checks
  • Successful completion and passing drug screen
  • Clear tuberculosis screening
  • A valid driver's license, no more than 5 points or DUI in last 3 years
