We are seeking an exceptional Lead Interoperability Engineer to spearhead our healthcare data interoperability initiatives. This strategic technical leadership role requires a seasoned professional who will lead our interoperability team in building and scaling cutting-edge solutions that enable seamless, secure, and compliant health data exchange across diverse healthcare ecosystems. The ideal candidate combines deep technical expertise in FHIR standards and healthcare APIs with proven leadership experience in managing high-performing data engineering and interoperability teams.
As a Lead of Interoperability Engineering, you will be a critical member of the Health Care Data Interoperability team responsible for building and maintaining real-time data pipelines and data infrastructure that connects to thousands of healthcare data sources around the country to bring together a person's health record in one place.
What You'll Do:
- Lead the design, development, and implementation of large-scale, real-time health data interoperability solutions supporting millions of patient records
- Architect and oversee Patient Access APIs, Provider Directory APIs, and Bulk FHIR APIs in compliance with CMS Interoperability and Patient Access Final Rule and lead the connections to national networks (Carequality, CommonWell, eHealth Exchange, TEFCA)
- Drive technical strategy for FHIR and US Core Implementation Guide adoption across all integration points
- Design scalable data pipelines for ingesting, transforming, and exchanging health data from multiple sources including EMRs, HIE/HINs, wearables, IoT devices, and remote patient monitoring systems
- Establish technical standards, best practices, and architectural patterns for interoperability solutions
- Lead evaluation and selection of interoperability platforms, tools, and vendor solutions
- Design, Architect, build, and maintain b.well's data real-time pipeline infrastructure using Python, Spark, Prefect, Kubernetes and applied LLMs / modern technologies
- Lead a team of data engineers to build data pipelines and infrastructure that connects to thousands of data sources around the country including health providers, insurance companies, pharmacies and labs.
- Help lead other developers to coach and improve their career development and amplify their impact
- You will safeguard sensitive data by following policies and training concerning your security and privacy responsibilities
- Collaborate with product, clinical, engineering, and business teams to define interoperability requirements
Requirements
Do you have experience in Technology security practices?, * 8+ years of hands-on experience in healthcare interoperability and health IT
- 5+ years of direct experience with FHIR standards and implementations (FHIR R4 required)
- 3+ years in a technical leadership role managing interoperability or data engineering teams
- Proven track record of leading large-scale, real-time health data exchange implementations serving 100K+ patients
- Extensive experience integrating with multiple EMR systems (minimum 5+ different EMR platforms)
- Direct experience implementing Patient Access APIs and Bulk FHIR APIs in compliance with CMS regulations and with HIE/HIN integrations and national networks (Carequality, CommonWell, or similar)
- Expert-level knowledge of HL7 FHIR R4 specification and resource models
- Deep understanding of US Core Implementation Guide, USCDI, and ONC certification requirements
- Proficiency with SMART on FHIR authorization framework (OAuth 2.0, OpenID Connect, SMART scopes)
- Strong knowledge of HL7 v2.x messaging standards (ADT, ORM, ORU, etc.)
- Experience with CDA (Clinical Document Architecture) and C-CDA documents
- Strong knowledge of API security: OAuth 2.0, JWT, mTLS, API keys, rate limiting
- Proficiency with message queuing systems (Kafka, RabbitMQ, AWS SQS, Azure Service Bus)
Technical Skills - Programming & Development:
- Strong programming skills in at least two of: Python, Java, JavaScript/TypeScript, Go, C#
- Experience with modern frameworks: Node.js, Spring Boot, .NET Core, Django/Flask
- Proficiency with SQL and NoSQL databases (PostgreSQL, MySQL, MongoDB, DynamoDB)
- Experience with data formats: JSON, XML, FHIR JSON/XML, HL7 v2.x, CSV
- Knowledge of data validation, schema validation (JSON Schema, FHIR Validator)

1.81.8 out of 5 stars Remote $175,000 - $210,000 a year, The target salary range for this position is $175,000 - $210,000 and is part of a competitive total rewards package including stock options, benefits, and incentive pay for eligible roles. Individual pay may vary from the target range and is determined by a number of factors including experience, location, internal pay equity, and other relevant business considerations. We review all employee pay and compensation programs annually at minimum to ensure competitive and fair pay.
